Change of Name or Address

Legal Name Change Policy听

Current students may change their name by听completing the听Personal Data Form听in the Student Center through听.听Appropriate state or court issued documentation that听indicates听a name change along with a valid photo identification card must be included with this form. This documentation can include but is not limited to the following:听

Marriage License听

Certificate of Naturalization听

Adoption/Birth Certificate听

Divorce Decree (stating听restored name)听

Court Order听

Alien Registration Card听

The documentation听submitted听must include the student鈥檚 new and former name.听

Preferred Name Change Policy听

Except when the use of an individual鈥檚 legal (or primary) name is required by听law,听current students may be听identified听by a preferred name.鈥� 糖心logo入口 wishes to allow its community to use a preferred听name to听give members an empowering, safe, and nondiscriminatory university experience.鈥� Every member of the campus community should feel empowered to be called by the name of their choosing.鈥� Equally important, no one should feel pressured to change their name for the acceptance or convenience of others.听

Policy: Current students can indicate a 鈥減referred鈥� first name, which is a name the student wishes to be commonly known as, if different from their legal name.鈥� A student may change their preferred first and/or middle names in their听.鈥� The student鈥檚 preferred name will be visible in the听following听areas, and may be subject to change:听

Class Rosters听

Grade Rosters听

Faculty Center - Class/Grade Roster听

Advisee Student Center 鈥� all tabs within advisee student services center听

Academic Requirements Reports听

Preferred names will not be used for official university record purposes such as enrollment verification, official transcripts, employment, payroll records, and, but not limited to, financial aid.鈥� Additionally, changing the preferred name will not change a student鈥檚 email address that appears in the student鈥檚 record.鈥�

Any attempt to misuse the preferred name policy for purposes of misrepresentation or fraud may result in a violation of the听Student Conduct Code听and may be referred to the Office of Student Life.
