General Instructions for CSUDH Students who have successfully met program completion requirements:

Select your completed CCPE Program from the buttons below
Fill out the entire form (including full name, complete mailing address, courses taken and their associated terms of completion, and your initials), and click Submit Application

Please allow 4-6 weeks* for program department review and processing. If you have any questions/issues with any of the request forms below, please notify eecertificate@csudh.edu.

* Certificates will only be mailed to addresses within the United States as we do not offer international shipping of CSUDH certificates. Certificate recipients can provide a valid U.S. mailing address which will ensure successful delivery, but any requests for shipment outside of the United States will not be processed.
